Every diabetic and hypertensive patient leaves with the same instruction: keep taking the medicine, check the reading, come back in three months. Most keep taking the medicine. Fewer check the reading. Fewer still come back on time, and the doctor only finds out when the HbA1c is worse than it needed to be.

This flow starts when a chronic condition is tagged on the record and the doctor sets a follow-up interval. At each interval the patient gets a message asking for the reading they were told to track: fasting sugar, blood pressure, weight, or whatever the doctor chose from the template.

The reply is filed as a structured value against the patient's record, not as a chat message someone has to copy across. When the review is due, the same thread offers the doctor's open slots and books one.

    Trigger

    The doctor tags the condition and sets the interval and the reading to track. Both can be dictated as part of the prescription.

    Message

    At the interval, a message goes from your clinic's number asking for the reading, in the language on the patient's record.

    Reply

    A number is filed. A number outside the doctor's threshold goes to the doctor the same day. Words go to the staff member you named.

    Record

    Readings appear as a trend on the record. When the review is due, the flow offers slots and books one. The visit note shows every check-in.

The things clinics ask first.

Anything with a reading or a question you want asked at an interval. Defaults ship for type 2 diabetes, hypertension and hypothyroidism. Doctors can add their own template for asthma, CKD or anything else.

The photo is attached to the record and the message goes to your named staff member to enter the value. The flow does not guess from an image.

No. The flow tells the patient that the doctor has been informed and to contact the clinic if they feel unwell. The clinical decision stays with the doctor, who sees the value straight away.

Yes. The flow messages the number on the record. Many elderly patients have a son or daughter's number listed, and that works.

It makes sure the visit happens. The reading between visits gives the doctor a trend to look at; the review is still booked and still in person.
