An EHR (Electronic Health Record) is a patient record designed from the start to be shared across multiple healthcare providers, not held by a single practice. Where an EMR is one clinic's chart, an EHR implies interoperability — a record that can travel with the patient.
In India, this is closer to what the Ayushman Bharat Digital Mission (ABDM) is building nationally: a patient's ABHA-linked health record, assembled from records shared with consent by every provider that has treated them. An individual clinic's software becomes part of this ecosystem when it supports ABDM properly — creating and pushing structured, FHIR-formatted records — even though the clinic's own software is technically still functioning as an EMR.
