An eye OPD produces structured data at every step: acuity on the chart, pressure from the tonometer, refraction from the optometrist, and the fundus finding. In a general EMR these become free text, and the glaucoma patient's IOP over two years is somewhere in a paragraph. The spectacle prescription is written on a slip for the optical shop and never sees the record.
